We aim to respond to all referral enquiries within 24 hours.

Step 1 — Referral Received
Commissioners, social workers or brokerage teams email referral details.

Step 2 — Initial Screening
We complete a desktop review and confirm suitability and availability.

Step 3 — Assessment & Visit
A full assessment is completed with the individual, family, and professionals.

Step 4 — Proposal & Hours Agreement
We provide a structured proposal including staffing model, hours, risks and mobilisation plan.

Step 5 — Transition Planning
We plan safe transitions with commissioners, health teams and families.

Step 6 — Move-In & 6-Week Review
 

Support plans are reviewed and adjusted to ensure stability and success.
